Sugar Low Hone Ki Alamat: When It Becomes an Emergency

Someone in trouble right now? Save both numbers.
Diabetes emergency guide

One high reading, repeated doses of insulin and a crash nobody saw coming. Here is what low blood sugar looks like, what to do first, and when to call 1122.

Man with eyes closed pressing his temples, showing the dizziness and headache that can come with low blood sugar

A man in his late twenties noticed his sugar was unusually high, took insulin to fix it, and ended up learning sugar low hone ki alamat the hard way.

He had not eaten anything sweet. In fact, he had barely eaten at all. So he did what made sense to him and took insulin. The number barely moved. He took more insulin and went off to exercise, hoping to force it down. Still no luck.

Eventually he landed in hospital, where the missing piece became clear: he was badly dehydrated. Once his fluids were replaced, his glucose finally started coming down. But those earlier doses had not magically disappeared just because the meter had been stubborn. The result? His glucose crashed dangerously low.

That is why low sugar should never be explained as “Sugar kam ho to kuch meetha kha lo.” Diabetes is not always that obedient.

Quick answer

What are the symptoms of low blood sugar in a diabetic, and when does it become a medical emergency? Early symptoms are shaking, sweating, sudden hunger, dizziness, and a racing heartbeat. It becomes an emergency when the person is confused, cannot swallow safely, has a seizure, or cannot be woken. If they are awake, give quick-acting sugar. If they are not, call 1122.

1Sugar Low Hone Ki Alamat Kya Hain?

Blood glucose below 70 mg/dL counts as hypoglycaemia. Below 54 mg/dL it is seriously low, and a low that leaves you too confused to help yourself is classed as severe.

Below 70 mg/dLHypoglycaemia
Below 54 mg/dLSeriously low
Too confused to help yourselfSevere

The number is only half the story, though. Someone sitting at 72 mg/dL and dropping fast after insulin needs more attention than someone steady at 72.

Early blood sugar low symptoms usually look like this:

  • Shaking and sweating
  • Sudden hunger
  • Dizziness or weakness
  • Racing heartbeat
  • Headache, irritability or anxiety
  • Blurred vision

As the brain runs short of fuel, things get stranger:

  • Confusion or unusual behaviour
  • Slurred speech
  • Poor coordination
  • Extreme sleepiness
  • Seizures
  • Loss of consciousness

Yes, someone acting drunk at a family function may not be drunk at all. If a person with diabetes suddenly seems “off”, check their glucose before you start guessing.

2Sugar Low Kyun Hota Hai?

Usually for boringly ordinary reasons:

  • A skipped or delayed meal after taking diabetes medicine
  • Too much insulin, or certain diabetes tablets
  • Exercise, which can keep lowering glucose for 24 hours or longer
  • Fasting in Ramzan without adjusting medicine
  • Correction doses of insulin that overlap, which is exactly what happened to our man in his twenties

3Diabetic Patient Mein Sugar Low Ho To Kya Karna Chahiye?

For sugar low ka ilaj in someone who is awake and can swallow, use the 15-minute rule recommended by the American Diabetes Association:

  1. Check glucose with a glucometer if you can.
  2. Take about 15 grams of quick-acting carbohydrate.
  3. Wait 15 minutes.
  4. Check again. If it is still below 70 mg/dL, repeat.
  5. Once it recovers, eat a small snack if your next meal is more than an hour away.

Half a cup (roughly 120 ml) of fruit juice is one of the easiest ways to do this. In Pakistan, that may simply mean a small packaged mango juice, but read the label because carbohydrate content varies. Diet juice will not help. Neither will “no added sugar” if there is barely any carbohydrate in it. Plain sugar stirred into water or glucose tablets also work.

Now the part everyone gets wrong. A low feels awful, your hands are shaking, and your brain is shouting “KUCH KHAO.” So half a cup of juice becomes a full carton, then biscuits, then someone arrives with mithai because the entire family has now joined the medical team.

Twenty minutes later the low is gone, and your glucose is heading past 250. Treat in measured amounts, wait, and recheck. Symptoms take a little longer to settle than the number does, so feeling shaky for a few minutes afterwards does not mean you need the whole kitchen.

4Sugar Low Kab Emergency Ban Jati Hai?

A low becomes a diabetic emergency when the person:

  • Is unconscious or cannot be woken
  • Has a seizure
  • Is too confused or drowsy to swallow safely
  • Keeps vomiting or does not improve after two rounds of treatment
  • Has breathing difficulty or deep, laboured breathing
Give nothing by mouth

In these situations, give nothing by mouth. No juice, no water, no honey, no sugar. The person can choke. Turn them onto their side, stay with them, and call SIEHS 1122 immediately.

This is not a hypothetical. SIEHS 1122 once received a call about a person everyone at the scene believed had died. When the team arrived and checked, his blood sugar was dangerously low. He had not died. He had passed out because of low blood sugar. That is why an unresponsive person with diabetes needs a glucose check and an ambulance, not assumptions.

SIEHS 1122 is a free emergency ambulance service in Sindh with 24/7 emergency ambulance response, and its emergency medical technicians provide care on the way to the hospital.

4 a.m. to 8 a.m.

Hormones released in the early hours can push glucose up before breakfast. This is called the dawn phenomenon.

5“Maine Kuch Khaya Hi Nahi, Phir Sugar High Kyun Hai?”

This is the question that started our man’s whole problem. Your glucose does not come only from food.

Your liver stores glucose and releases it between meals, which is how you survive eight hours of sleep without someone feeding you every two hours. It can also make new glucose from scratch, a process called gluconeogenesis. The name sounds designed to ruin an otherwise readable article, but the idea is simple: your body can make glucose even when you have not eaten a single carbohydrate. Your liver would like a word with anyone who says, “But I haven’t eaten anything sweet.”

Morning highs work the same way. Hormones released in the early hours, often between about 4 a.m. and 8 a.m., can push glucose up before breakfast. This is called the dawn phenomenon.

Three things at once

Dehydration, repeated insulin doses and a workout, all acting on one body.

6Back to the Man in His Twenties: What Actually Went Wrong

Several things were happening at once:

  • His glucose was high, but not because of food.
  • He was dehydrated. Less water in the blood means more concentrated glucose. High sugar also makes you pass more urine, so you lose more water. Then the sugar concentrates further. It is a vicious circle.
  • He kept correcting with insulin. Rapid acting insulin keeps working for roughly three to five hours. Doses at 2 pm, 3 pm, and 4 pm overlap, a problem called insulin stacking. The first dose did not fail. It was still on duty.
  • He exercised. Working muscles use glucose, and insulin sensitivity can stay raised for 24 hours or longer.
  • He judged everything from one number, not the direction it was travelling.

Then the fluids went in, the dehydration cleared, and all that insulin finally had room to work.

Each correction dose keeps working for roughly three to five hours, so doses at 2 pm, 3 pm and 4 pm overlap. This is insulin stacking, shown here as an example.

7Read the Arrow, Not Just the Number

If you use a continuous glucose monitor, 95 mg/dL with a flat arrow is a very different story from 95 mg/dL with two arrows pointing down. Sensors also lag behind actual blood glucose during rapid changes, so if the screen says you are fine, but you are sweating and shaking, trust your body and confirm with a fingerstick.

Most people in Pakistan use a glucometer, so the same principle applies: one reading is a snapshot, not the film. Ask what you ate, what medicine you took, when, and what you did since.

Very hard to wake?

Check their glucose. Do not assume deep sleep.

8Night Time Lows and Silent Lows

“Sugar low hogi to meri aankh khul jayegi” is a comforting thought, but not always true. Warning signs of an overnight low include sweaty bedding, nightmares, restless sleep, a morning headache, or unusual exhaustion. If someone with diabetes is very hard to wake, check their glucose. Do not assume deep sleep.

Some people also stop feeling the early hypoglycaemia ki alamat altogether. This is called hypoglycaemia unawareness, and the first sign may be confusion. Repeated unexplained lows deserve a conversation with a doctor.

9Sugar Low in Ramzan

Fasting raises the risk of lows, especially for people on insulin or certain tablets. Speak to your doctor before Ramzan about adjusting doses. If glucose falls below 70 mg/dL during a fast, breaking it is generally advised. Iftar can wait. Your brain cannot.

10Can You Get Sugar Low Without Diabetes?

Yes, though less commonly. Long gaps between meals, prolonged fasting or, rarely, reactive hypoglycaemia after a very sugary meal can cause the same symptoms. Treat it the same way and see a doctor if it keeps happening. For non emergency questions, you can speak to a doctor through the Tele Tabeeb 1123 telemedicine service.

Sugar Low Hone Ki Alamat: The Story Behind the Number

Knowing sugar low hone ki alamat can stop a mild low becoming an emergency. But a person can be high without eating sweets, correction insulin can still be working when the meter looks stubborn, and too much juice can send glucose racing the other way.

So when the number looks strange, do not throw insulin, exercise, or an entire carton of mango juice at it. Ask what happened in the last few hours, and where the glucose is heading. The number matters, but the story behind it can matter more.

Save 1122 on every family phone, especially where someone has diabetes.

Frequently asked questions

1. Sugar low hone ki alamat kya hain?
